ABSTRACT

Introduction: Cervical cancer (Ca Cx) is the fourth most frequent cancer in women with an estimated 57000 new cases in 2018 representing 6.6% of all female cancers. Approximately 90% of deaths from cervical cancer occurred in low- and middle-income countries. Material and Methods: A retrospective radiotherapy treatment planning comparative study conducted at the Department of Radiation Physics, Kidwai Memorial Institute of Oncology, Bangalore during June 2018- March 2019. Result: All the plans were normalized to 100 % at Target mean to achieve a similar target dose for quantitative comparison of DVHs. The results for target coverage, OAR sparing, integral dose, and monitoring units. Conclusions: The tradeoff of using 6 MV and 18 MV for cervix patients depends on many parameters. Since the same PTV coverage was forced for both energies by having the same optimization constraints, there was little difference in target coverage and conformity index for both energies.

ABSTRACT

BACKGROUND: This study sought to evaluate the long-term recurrence rate of atrioventricular nodal reentrant tachycardia (AVNRT) after radiofrequency catheter ablation. The clinical and electrophysiological features of patients with AVNRT and their immediate outcomes after undergoing slow pathway ablation/modification were also studied. METHODS AND RESULTS: The study included 264 consecutive patients with AVNRT (mean age 46 -/+ 15 years, 143 women, 121 men) who underwent slow pathway ablation/modification using a combined electrophysiological and anatomical approach. The primary endpoint of ablation procedure was non- inducibility of the arrhythmia. The primary endpoint of the study was the recurrence of AVNRT on follow-up. Acute success was achieved in 262 (99.6%) patients. Complication rate of the ablation procedure was 2.6% and the average fluoroscopy time was 18.3 -/+ 11 minutes. The patients were followed up for a mean duration of 20 -/+ 9 months during which there was only one case of recurrence. CONCLUSION: Radiofrequency ablation or modification of slow pathway is highly effective in the treatment of AVNRT. The technique has a high initial success rate and a low complication rate. The recurrence rates are extremely low (0.3%) on long-term follow-up.

ABSTRACT

The molecular forces involved in protein-nucleic acid interaction are electrostatic, stacking and hydrogen-bonding. These interactions have a certain amount of specificity due to the directional nature of such interactions and the spatial contributions of the steric effects of different substituent groups. Quantum chemical calculations on these interactions have been reported which clearly bring out such features. While the binding energies for electrostatic interactions are an order of magnitude higher, the differences in interaction energies for structures stabilised by hydrogen-bonding and stacking are relatively small. Thus, the molecular interactions alone cannot explain the highly specific nature of binding observed in certain segments of proteins and nucleic acids. It is therefore logical to assume that the sequence dependent three dimensional structures of these molecules help to place the functional groups in the correct geometry for a favourable interaction between the two molecules. We have carried out 2D-FT nuclear magnetic resonance studies on the oligonucleotide d- GGATCCGGATCC. This oligonucleotide sequence has two binding sites for the restriction enzyme Bam H1. Our studies indicate that the conformation of this DNA fragment is predominantly B-type except near the binding sites where the ribose ring prefers a 3E conformation. This interesting finding raises the general question about the presence of specificity in the inherent backbone structures of proteins and nucleic acids as opposed to specific intermolecular interactions which may induce conformational changes to facilitate such binding.
